How to claim the benefits

  1. For the memorial service, simply call the EProtect Service Hotline. The family member needs to submit the following:
    • Death Certificate (Certified True Copy)
    • EProtect Card or Certificate of Membership
    • Birth Certificate of the deceased (Certified True Copy)
  2. For the accidental cash benefits, the following additional documents are required:
    • Police Report or Incident Report duly notarized
    • Marriage Certificate (if married)
    • Birth Certificate of the beneficiary (Certified True Copy)
Note: The benefits are not transferable. The company has the right to retrieve claimed benefits for cases of fraudulent or disapproved claims.

Declinable Areas

Below list enumerates the areas that are not acceptable for EProtect coverage. Applications coming from any of these shall be declined:
LOCAL AREAS : Ifugao (except Lagawe, Lamut, Banaue & Alfonsolista), Lanao Del Sur (except Marawi City), Sultan Kudarat (Columbio, Kalamansig, Lebak, Lutayan, Palimbang), Basilan, Maguindanao (Ampatuan, Bararira, Buldon, Datu Paglas, Datu Piad, Gen. S.K. Pendatun, Kabuntalan, Maganoy – Shariss Aguak, Matanog, Pagalungan, Sultan sa Barongis, Talayan), North Cotabato (Alameda, Aleosan, Antipas, Arakan, Carmen, Magpit, Pikit, Pres. Roxas), Davao Oriental (Banganga, Caraga, Manay, Tarragona), Davao Del Sur (Don Marcelino, Jose Abad Santos, Sarangani), Sulu (all areas) and Tawi-tawi.
Likewise, all claims against unprovoked murder and assault arising out in any of the following places / locations shall also be declined:
LOCAL AREAS : Lanao del Norte, Lanao del Sur, South Cotabato, North Cotabato, Maguindanao, Sultan Kudarat, Sulu Archipelago, Zamboanga Sibugay Municipalities, Zamboang del Sur Municipalities (EXCEPT Zamboanga City) Zamboanga del Norte Municipalities.
FOREIGN TERRITORIES : Iran, Iraq, Libya, Lebanon, Israel, Jordan, Kuwait, Syria, Africa, Nigeria, India and Pakistan.

Terms and Conditions

This EProtect card entitles the Member whose name has been registered in Pinoy SpringBoard (PSB) to a complete memorial service upon death. If death is due to accident, the Member gets an additional cash benefit. Death due to illness occuring within 180-days from date of activation is not covered.
By activating this card, the MEMBER affirms that he is not below 10 years old nor above 65 years old at the time of card activation. He is a Filipino citizen with a Philippine residence but not in any of the declinable areas in PSB website, brochures, and other related publications. His occupation is not in any way similar or categorized as policeman, politician, journalist, media personality, army personnel, or seaman/seafarer. He is not inflicted with Cancer or HIV. He is aware of a maximum limit of two (2) EProtect units in any given year.
The Member further affirms that he is aware of the other terms and conditions of this card including the guarantees, benefits, and service procedures governing the EProtect which are fully disclosed by consultants and provided in the website and in all PSB's sales materials.
The coverage signified by this card is valid for a period of one (1) year from date of activation and shall terminate upon availment of any of the benefits herein specified.
The Company shall become liable for the compensation under ACCIDENTAL TOTAL AND PERMANENT DISABILITY, if permanent disablement occurs within twelve calendar months of bodily injury and not followed within twelve calendar months of the said bodily injury by the death of the insured.

Exclusion in Memorial Service Benefit

Please be informed of the exclusions in EProtect memorial service benefit, as follows:
1. The EProtect coverage is expired.
2. The age of the member is less than 10 years old or more than 70 years old at the time of death.
3. Death is due to cancer or HIV. (For activation date Feb 15, 2010 and onwards)
4. Death is due to coronary artery disease, diabetis mellitus and kidney disease and the plan coverage is still within 180 days from the activation date. (For activation date April 15, 2010 and onwards)
5. Death is due to other illnesses and the plan coverage is still within 180 days from the activation date.
6. Death is due to unprovoked murder and assault that occurs in any of the listed declinable areas or war zones as follows:

a) Lanao del Norte     b) Sultan Kudarat
c) Lanao del Sur        d) Sulu Archipelago
e) South Cotabato     f) Zamboanga Sibugay Municipalities
g) North Cotabato     h) Zamboanga del Sur Municipalities (EXCEPT Zamboanga City)
i) Maguindanao          j) Zamboanga del Norte Municipalities
7. Death or disability while the member is engaging in or practicing for or taking part in any of the excluded activities:

a) hunting                b) football
c) racing of all kinds  d) yatching
e) steeple chasing     f) scuba or snuba diving ( snorkeling with scuba diving)
g) polo playing         h) parachuting
i) mountaineering      j) skydiving
k) winter sports        l) boxing
m) ice hockey           n) wrestling
o) football                p) other similar activities
8. Death or disability consequent upon the insured committing or attempting to commit suicide (whether sane or insane) or willful exposing himself to needless peril except in an atempt to save human life.
9. Death due to war, invasions, act of foreign enemy, hostilities, civil war, rebellion, revolution, insurrection or military or usurped power, the insured engaging in or involving in military or para-military activities, active or direct participation in strike, riot, and civil commotion, any weapon or instrument employing atomic fission or radioactive force, whether in time of peace or war.
10. Death or disablement consequent upon or contributed by the member/insured person:
  • having taken a drug unless the Insured proves that the drug was taken in accordance with proper medical prescriptions and directions and not for treatment of drug addiction
  • suffering  from pre-existing physical or mental defect or infirmity which had not been declared to and accepted in writing by the company
  • while under the influence of alcohol or unprescribed drugs unless it can be established that alcohol or drugs was not a factor contributing to the happening of the injury
11. Death or disablement consequent upon or contributed by the member/insured person being insane or pregnant
12. Acts of God or natural calamities such as earthquake, flood, typhoon, volcanic eruption, tidal wave, etc.
